CPC online exam in UAE

Does the video calling app used by AAPC in the CPC exam called EXAMITY requires me to use VPN? In UAE video calling app is not allowed, if I want to access these app can AAPC allow me to use VPN?

64718 with 24305?

Good afternoon,

Based on NCCI edits, code 64718 (column 2) is bundled into 24305 (column 1), however the RVU for code 64718 is higher than that of 24305.

Is there ever a scenario where both codes are billed for the same arm but 64718 is reimbursed over 24305?

Appendectomy vs colectomy

We are coding a surgery for a Laparoscopic appendectomy with partial cecectomy. Based on the clinic note, both procedures were planned due to an appendiceal orifice polyp. We have two different opinions. Should this be a 44970 Laparoscopic appendectomy or 44204 (52) Laparoscopic colectomy, partial, with anastomosis? Or??
Any input would be greatly appreciated. Thank you

Screening vs Surveillance Colonoscopy

Our office and facility need clarification on colonoscopy coding for polyp surveillance. Some are saying it should be a high risk screening due to polyps (CPT 45378, DX: Z12.11, Z86.010) and patient's insurance will pay 100%. The other opinion is that it should be a surveillance colonoscopy due to polyps (CPT 45378, DX: Z86.010) and patient's deductible/co-insurance would apply.

Cannot find specific CMS guidelines in regards to this. Any help would be appreciated!!

Denial for 90471 when billed with 90746 and 86580

Hello,

I am currently receiving denials from Select Health for the 90471 code even though it is billed with an immunization (90476) if 86580 is also on the claim. They are denying it for subset procedure disallow. Has anyone come across this and know anything about fixing it? I have just been receiving these denials from Select Health for the past couple of weeks. Thanks!

Decision For Surgery Modifier (57)

We frequently have a scenario similar to the one below and needing advice to get all visits paid.

09/01/2020 Admit(further workup ordered) CPT: 99222
09/02/2020 Subsequent Visit (Decision for surgery) CPT: 99232
09/03/2020 Surgery CPT: 44140
